In order to form your own team/join a team, below is what you will need:
- Teams must have a minimum of 8 players.
- Each team must have a Certified Coach (someone who has been through the CYO Coach’s Development program, is not a student, and is a minimum of 19 years old.
- Per player fee is $117.00 or $930 per team.
- Payment must be made by 1:00 pm, Friday November 9th. Checks made payable to Diocese of Cleveland CYO – Deliver payment to STVM office of the Athletic Director
- All participants must provide a current sports eligibility physical and an emergency medical form (available at https://ccdocle.org/ ) Current means conducted after March 31st, 2018.
- Team lists and coaches’ information can be given to Junior Elliot Whitmyer, also due by November 9th
